Consent and Agreement for Use of BS-STEAM Resources

Bay Shore STEAM Corporation (BS-STEAM) provides communication platforms, equipment, mentors and volunteers, funding, and event and travel support to students in the programs it supports, including FIRST Robotics Competition Team 271 and the Manufacturing Club. This agreement explains the rules for using those resources and asks parents, guardians, and students for their consent.

Current version: Version 2.0, October 1, 2026. It replaces the earlier Communication Waiver.

What You Are Asked to Decide

The signature page has three sections. A parent or guardian marks and initials each one, and the student initials it too.

A. Digital Communication

Accounts on the BS-STEAM platforms, currently Mattermost and Nextcloud, and the use of BS-STEAM electronic devices such as laptops. Agree or decline.

B. Media Release

Whether BS-STEAM may use photographs and recordings of your child. Three choices: no; images without name; or images with first name and last initial.

C. Tools, Travel, and Emergency Care

BS-STEAM-owned tools, machines, and devices, travel and lodging that BS-STEAM books, and activities BS-STEAM itself runs. Includes the injury waiver and the release of claims. Agree or decline.

Good to Know

  • Each section is optional. Declining a section affects only what that section covers. It does not prevent a student from participating in Team 271 or any other school club or class.
  • Mentoring and funding never depend on it. Mentoring by BS-STEAM volunteers and BS-STEAM’s funding of a District program do not depend on this agreement.
  • This is BS-STEAM’s agreement. BS-STEAM is an independent nonprofit. The Bay Shore Union Free School District is not a party to the agreement, and it does not replace the District’s or FIRST’s forms.
  • Signed each school year. BS-STEAM asks families to sign again at the start of each school year. A student who turns 18 signs in their own name.
  • You can withdraw at any time. Send written notice to Info@BS-STEAM.com from the email address on file.

How to Return It

Return the signed agreement to any BS-STEAM staff member, or email a scanned copy to Info@BS-STEAM.com.

To report a concern, you can write to Info@BS-STEAM.com or to any member of the Board of Directors. The agreement lists other ways to report as well.
